Philips Respironics BiPAP A series ventilators: alarm malfunction and risk of therapy interruptions in ventilators not intended for life-support, DSI/2024/006

Philips Respironics has issued a Field Safety Notice (FSN) relating to the Bilevel Positive Airway Pressure (BiPAP) A series ventilators. This relates to a Ventilator Inoperative alarm which could result in the potential loss of therapy to patients without warning.

Device Name: BiPAP A30, BiPAP A30 EFL, BiPAP A30 Hybrid, BiPAP A40, BiPAP A40 EFL, BiPAP A40 Pro

Affected lot numbers/serial numbers: All devices

Manufactured by: Philips Respironics

Explanation of identified safety issue

Philips BiPAP ventilator devices are primarily used to treat patients with obstructive sleep apnoea (OSA), respiratory insufficiency or respiratory failure. These devices are not intended for life support (patients who are dependent on artificial ventilation for their immediate life support) as indicated in the device instructions for use (IFU).

In May 2024, Philips released an FSN relating to an error causing interruptions or loss of therapy in the above named BiPAP ventilators.

An internal error in affected devices will trigger a Ventilator Inoperative alarm and the device will shut down if the cause of the error indicates the device cannot deliver therapy. The alarm silence button will flash red, and a message will appear on the device screen displaying “Ventilator Inoperative”. Please refer to the FSN for an example of what this message looks like.

This alarm can occur when 3 device restarts have occurred in a 24 hour period or it can occur spontaneously without a previous restart.

Interruption and / or loss of therapy may lead to adverse events including respiratory insufficiency or potentially death.

Interruption of therapy may present as symptoms of:

• nausea and vomiting,
• fragmented and unrefreshing sleep,
• tiredness (fatigue) or lethargy,
• shortness of breath,
• increased effort to breathe,
• slow, shallow or laboured breathing,
• bluish skin, lips or nails (cyanosis),
• coughing and wheezing,
• unusual jerking or shaking movements.

In the context of a worldwide use of over 100 million potential uses, the overall likelihood of a malfunction occurring is very low. At the current time, there have been 888 reports worldwide, 10 reports of suspected serious injury and 7 reports of death associated with this issue.

The MHRA has received reports of incidents related to an issue with the Ventilator Inoperative alarm in these devices and are continuing to investigate these reports in collaboration with the manufacturer Philips Respironics.

Any changes to the current advice will be communicated to users as soon as possible and posted onto our website.

Users of these devices should also be aware of a second FSN , released by Philips Respironics in July 2024, regarding an alarm malfunction linked to the oxygen sensor inside specific model numbers of the A30 EFL, A40 PRO and A40 EFL ventilators. This issue does not cause interruptions or loss of therapy and is therefore of lower risk to patients than the Ventilator Inoperative alarm issue covered in this DSI. For further information on this separate issue and the affected model numbers, please refer to the manufacturer’s FSN.

Actions for healthcare professionals

• Identify patients using the relevant devices listed in the FSN in both inpatient and community-based settings
• Assess if patients can tolerate interruptions to their therapy.
• If patients cannot tolerate interruptions of therapy, prioritise and provide an alternative device appropriate to their level of ventilation dependency as soon as possible depending on local resources.
• If patients can tolerate interruptions in therapy, consider the benefits and risks of continued use on an individual basis. Prioritise patients at higher risk to ensure an alternative device is provided at the earliest opportunity depending on local resources. Lower risk patients should also be provided with an alternative device when possible depending on local resources.
• For patients using this device who have a backup device available, advise the patients / carers or caregivers or hospital staff that if the alarm occurs to immediately remove the device, and connect the patient to an alternative device.
• Ensure community-based patients/carers or caregivers are provided with clear instructions for changing over their device.

As an additional measure, a forced restart of the device may temporarily restore function of the device if it has entered the inoperable state whilst waiting for a replacement device:

• Power off the device by pressing the start/stop button. If the ventilator screen displays the “Power off” command, then press the “Yes” button to shut off the device and silence the alarm
• Unplug the power cord from the wall or from the device itself
• Remove the battery from the ventilator device. If a detachable battery pack is used, open the battery compartment at the top of battery module accessory and lift out the battery using the release lever on top of the battery. If an external battery pack is used, unplug the battery pack cord from the back of the ventilator
• Leave the battery disconnected from the ventilator for at least 30 seconds
• Reconnect the applicable battery in use to the ventilator
• Plug the power cord back in to the wall or the device itself
• Power on the ventilator by pressing the Start/Stop button
• Once the ventilator powers back, therapy may be restarted

There are specific reporting arrangements for healthcare professionals to follow in each region. Healthcare professionals should report incidents:

• in England and Wales to the Yellow Card scheme or via the Yellow Card app
• in Scotland to Incident Reporting & Investigation Centre (IRIC) and their local incident recording system
• in Northern Ireland to the Northern Ireland Adverse Incident Centre and their local incident recording system

Actions for patients / carers or caregivers

• Patients / carers or caregivers using these devices or caring for a patient who is, should follow the advice of the patient’s healthcare professional who will make recommendations for treatment and may recommend an alternative device.
• Patients / carers or caregivers using these devices or caring for a patient who is, should be aware of possible symptoms associated with loss of therapy (see above list of symptoms).
• If the ‘Ventilator Inoperative’ alarm occurs, immediately remove the device and connect the patient to an alternative device if available. Contact your home care equipment provider for advice and / or an alternative device.

As an additional measure a forced restart of the device may temporarily restore function of the device while waiting for a replacement device:

• Power off the device by pressing the start/stop button. If the ventilator screen displays the “Power off” command then press the “Yes” button to shut off the device and silence the alarm
• Patients / carers or caregivers that have concerns about continued use of these devices should discuss this with the patient’s healthcare professional.
• Seek urgent medical attention if patients feel unwell following the interruptions of therapy from the malfunction of these devices whilst awaiting a replacement.
• Any patients experiencing a malfunction of this device and / or feel that they have been harmed by a medical device should inform their care team at the earliest possible opportunity and then report this via the Yellow Card scheme .

Actions for distributors

• Review your inventory and determine if affected ventilators are present in your stock
• Quarantine affected stock and contact Philips Respironics for instruction
• Distribute a copy of the FSN to customers with affected ventilators
• Complete the response form attached to the FSN and return it to Philips Respironics

Behind the Brand: Landyachtz

For countless skaters, the simple act of seeing someone riding past them one day has been enough to spark their imagination and inspire them to get on a board themselves and begin a lifetimes of side stance action.

Especially for  Landyachtz Co-Founders, Thomas Edstrand and Michael Perreten. Check out how the pair created one of the biggest brands in the history of longboarding in this Behind the Brand .

For countless skaters, the simple act of seeing someone riding past them one day has been enough to spark their imagination and inspire them to get on a board themselves and begin a lifetimes of side stance action. In the case of Thomas Edstrand, this common occurrence follows a similar trajectory. Except when Edstrand came across Michael Perreten on the University of Victoria campus, the two would transform their ensuing conversation over Perreten’s food coloring-stained, maple and fiberglass-wrapped deck, into a pursuit to start Landyachtz in the basement of a ski cabin in Whistler, British Columbia.

With a backdrop of steep, hilly terrain, the pair began one of the biggest companies in longboarding history with an idealistic goal: creating boards that were strong and technically advanced, in hopes of making it to the bottoms of their local hills in one piece. To conquer the challenge, Perreten assumed the role of shape design and construction while Edstrand took over the art and graphic design end of the operation.

Perhaps most central to the history of Landyachtz is the success they had in drop mounting construction before anyone else in the industry caught on. From the initial release of the Landyachtz DH Race board, the team crafted one of the first boards where the baseplate of the trucks sat mounted atop the deck while the truck’s hangar dropped through the deck, keeping the trucks as close to the ground. In time, the boys followed up with the Landyachtz Evo as the first production drop foot platform, providing unparalleled advantages in the downhill racing world and essentially changing the game. The Evo’s design came complete with an upturned nose for responsiveness in the front of the board while a detuned tail in back of the board provided stability and created the ability to tuck fast, handle corners and drift into speed checks.

In another sense, Landyachtz gained notoriety for their superior competence in using carbon fiber in their builds. Used to provide the durability and stiffness to fly down the sketchiest of mountain hills, the incorporation of carbon fiber also made for boards that were heavy enough to withstand the style of riding they became synonymous with. In the same sense, they were able to take the carbon fiber and infuse it in their trucks and boards designed for other disciplines.

Though Landyachtz has their roots in fast, speed-handling downhill boards, they have made significant contributions across other disciplines of skateboarding by taking their technical strengths and incorporating them into their diverse lineup of cruiser boards. In specific example, the Landyachtz Dinghy has benefitted from years of R&D in the way it has been refined over the years.

Unlike other similarly styled cruisers that have typically been produced with generic molds, these boards received their own specific molds over the years and have had the benefit of meticulous attention to detail throughout their design process. Combined with strength-engineered trucks, freeride-inspired, stoneground finish wheels and bearings with built in spacers, the Dinghy stands as a solid example of Landyachtz’s aptitude in downhill applied to other boards across the spectrum. 

Of course, another key behind the success of Landyachtz is the exemplary riding showcased by their team members. From the all terrain ripping of guys like Billy “Bones” Meiners to the outspoken enthusiasm of scene supporters like Bricin “Striker” Lyons , the Landyachtz team provides the perfect examples of how to get down on their boards and have fun doing it. Often, team riders have started there relationship with the brand as local allies who grow their skills and interactions with the brand in tandem. While strong riding is an obvious pillar of the team, the Landyachtz squad is more accurately measured by their personality and contributions beyond skill level. Those who have meshed well with other team riders and aligned with the brand vision have been able to notch a spot on the coveted lineup and enjoy that perks that come with it. 

This architecture deserves appreciation but Cassandre Lemoine's no complys are even more impressive. 

However, Landyachtz supports more than just the team that keeps their boards in the streets. Over the years they have placed a special emphasis on supporting events and humanitarian causes, both locally and internationally. In their homeland of Canada, their continued efforts in keeping the well-attended, Attack of Danger Bay alive have helped it become one of the longest running downhill skate events in the world. Likewise, their involvement in events like the Toronto Board Meeting and various college tours helps maintain a presence in general industry events and beyond.

Alex Hannigan's mullet may be under his helmet but his riding is still all business in the front. 

Away from home, Landyachtz has remained supportive of important environmental initiatives as well, most notably including their One Board, One Tree campaign . With this initiative, the company funds the planting of one Canadian maple tree for every board they sell as a way to directly give back to the hands that feed the business. Under the notion that planting one tree can in turn provide enough wood for up to 60 skateboards, they have worked with a handful of different organizations that have been responsible for planting over 120 thousand trees so far.

With one of the most storied pasts in longboarding, there’s no doubt that Landyachtz will continue to build on their established position in the industry with new product offerings and initiatives to help support the industry. As is typical, their new boards will go through a two year cycle that includes development and release of an original graphic, followed by a year of letting the boards out in the market and adding a graphic revision. Then, when the time is right, additions to the line will be made and the cycle will reset. With some of the most commercially successful boards of all time having been released under the Landyachtz umbrella, it’s anyone’s guess as to which of their upcoming releases will be the next to go down in the history books.
